Optimale tippositie met intra-atriale ECG-techniek
Centrale veneuze toegangssystemen (in het Engels Central Venous Access Devices of CVAD) worden gebruikt voor korte of lange termijn infusie van vloeistoffen of wanneer een perifere veneuze toegang niet mogelijk of moeilijk is.
Centraal veneuze toegang
Centraal veneuze toegangen kunnen zijn:
- Centraal veneuze katheters (CVC)
- Volledig geïmplanteerde poortkatheters (PAC)
- Perifeer ingebrachte centrale katheters (PICC)
De juiste positie van de tip van de centrale katheter is essentieel voor de optimale functie van de katheter.
Tippositie met intra-atriale ECG
Met verkeerde plaatsing van centraal en perifeer ingebrachte centraal veneuze katheters wordt voornamelijk de onjuiste locatie van de kathetertip bedoeld.2,3 Een optimale kathetertippositie van een CVAD is belangrijk aangezien een verkeerde positie van de kathetertip wordt geassocieerd met een verhoogd risico op complicaties. Deze complicaties kunnen optreden op het moment van inbrengen of na inbrengen.
Mogelijke complicaties tijdens het inbrengen:
- Atriale fibrillatie
- Ventriculaire tachycardie of andere aritmie
- Atriale wandbeschadiging
- Slecht functioneren van de katheter
Mogelijke complicaties na het inbrengen:
- Disfunctie van de katheter
- Vaaterosie
- Veneuze trombose
Bovendien betekent verkeerde plaatsing niet alleen onjuiste plaatsing in de venen, maar ook katheters die worden ingebracht in extra-veneuze posities zoals in het arteriële systeem, mediastinum, pleura, trachea, oesofagus, enz.4
Zoals beschreven door verschillende auteurs1,3,18 bevindt de kathetertip van een CVAD zich idealiter in een "veilig gebied" dat zich uitstrekt van 2 cm boven tot 2 cm onder de cavo-atriale junctie omdat:
- het uiteinde van de katheter zich in een gebied met maximale bloedstroom bevindt,
- er geen direct contact is met de aderwand (parallel aan de aderwand),
- deze positie rekening houdt met mogelijke bewegingen van de katheter.
De juiste tippositie van de katheter moet al gecontroleerd worden tijdens de plaatsing.
- gebruik ultrasound om de meest geschikte vene te selecteren,
- de intra-atriale ECG techniek wordt gebruikt om de tippositie van de CVAD te bevestigen tijdens plaatsing en na plaatsing,9,10,18
- intra-atriale ECG-tippositieplaatsing met Certodyn® en Alphacard® is nauwkeurig, veilig en toepasbaar bij alle volwassen en pediatrische patiënten.9,18
ECG-techniek is even nauwkeurig als fluoroscopie maar heeft veel voordelen: het is eenvoudiger, gemakkelijker beschikbaar, minder duur, veiliger en meer kosteneffectief.2
ECG-techniek stelt de plaatser in staat de positie van de kathetertip onmiddellijk en al tijdens de procedure te verifiëren. Noch de patiënt noch de chirurg wordt aan straling blootgesteld en de patiënt wordt niet onnodig onderworpen aan de potentiële risico's van contrastvloeistof (nefrotoxiciteit, allergie).
Het gebruik van intra-atriale ECG tijdens de plaatsingsprocedure verlengt de inbrengingstijd niet. Dit in tegenstelling tot fluoroscopie/radiologie, dat vaak om logistieke redenen, wel gepaard gaat met een lange proceduretijd.18
