Convex stomamateriaal – een oplossing voor jouw patiënten?

Stomacomplicaties kunnen zich binnen een maand na de operatie voordoen, maar ook op een later moment. [1] Lekkage kan verschillende oorzaken hebben, zoals de plaats van de stoma, verandering van de stoma, of een stoma onder of op huidniveau ligt, fistelvorming en ook als er sprake is van grote hoeveelheden vloeibare ontlasting.. [1] Wanneer een van deze obstakels zich voordoet en het stomamateriaal niet goed aansluit kan ontlasting of urine de peristomale huid irriteren en zelfs onder de stomahuidplaat sijpelen. [2] Als de huid eenmaal geïrriteerd is, kan dat voor huidproblemen zorgen waardoor de huidplaat minder goed hecht wat ook weer lekkage veroorzaakt. [3]

Als een patiënt last heeft van stomalekkage, kan een convex stoma huidplaat het probleem oplossen. Bepaal samen met je patiënt welk type en vorm convex huidplaat comfortabel op de huid past, goed aansluit en een prettige pasvorm heeft. Kijk ook naar het type stoma en de contouren van de buik van de patiënt.

Convex stomazakjes kunnen helpen om stomalekkage te voorkomen  

Het doel van een convex stomazakje is te zorgen voor een goede afdichting van het stomamateriaal op de omringende huid om lekkage van de stoma te voorkomen, één van de meest voorkomende problemen voor stomadragers. [4,5] voor veel stomapatiënten. Convex stomamateriaal is ontworpen om [6,7,8,9]

  • voldoende druk uit te oefenen rond de stoma op de peristomale huid, met name in plooien en huidplooien, zodat de huidplaat goed aansluit en zodat de stoma goed omhoog wordt gedrukt en verder in het zakje steekt, waardoor er geen ontlasting of urine onder de huidplaat komt; 
  • voldoende gelijkmatige druk te geven zonder de omringende huid te beschadigen;. Dit verbetert de afvoer van stoma‑output naar het zakje en is vooral handig bij stoma's op huidniveau of wat dieper gelegen stoma’s
  • accessoires zoals convex ringen overbodig te maken.

De keuze voor zachte convexiteit kan extra voordelen bieden [8,9]

  • Een soft convex huidplaat heeft de voordelen van convexiteit zonder overdreven druk of ongemak te veroorzaken maar geeft wel een goede afdichting.Het zachte materiaal is comfortabel voor de huid en past zich aan de lichaamscontouren aan.  .
  • Hierdoor kan een soft convex huidplaat voor je stomapatiënt nog comfortabeler zijn om te dragen.
     
Wanneer adviseer je convex stomamateriaal?

Elke stoma is anders en vaak kan convexiteit een grote hulp zijn. Om het juiste hulpmiddel te kiezen, moet het hulpmiddel zowel aan de anatomische omstandigheden en dynamiek van de buik van de patiënt als aan de kenmerken van zijn stoma worden aangepast.Kijk ook wat er gebeurt als de stomadrager staat, ligt, buigt of bukt. Daarom verschillen convexe stomasystemen onder andere in hoek, vorm en diepte van de convexiteit.. [10]

Uiteindelijk is het jouw klinische ervaring die bepaalt of je een convex stomahuidplaat aanbeveelt aan je patiënt en welke eigenschappen de oplossing moet hebben.

Convex stomamateriaal is geschikt voor: [7]

  • Flush stoma: Een stoma moet 2,5 cm lang zijn, zodat de afdichting rond de basis van de stoma goed kan werken. Als een stoma echter ter hoogte van de buikhuid zit, wordt deze als flush beschouwd.
  • Ingetrokken stoma: een ingetrokken stoma zit onder het niveau van de huid. Dit kan worden veroorzaakt door een technische moeilijkheid tijdens de operatie of door postoperatieve gewichtstoename.
  • Telescopische stoma: Vooral 's nachts, wanneer de patiënt in rugligging ligt, kan de telescopische stoma naar binnen of onder huidniveau glijden.
  • Slecht geplaatste stoma: als een stoma werd aangelegd tijdens een spoedoperatie terwijl de patiënt rugligging had of een opgezwollen buik had, kan het resultaat niet optimaal zijn.
  • Oneffen peristomale huid: de huid kan kuiltjes en plooien vormen die lekkage veroorzaken.
  • Zachte buik: een buik met zwakke spierspanning en zachte loszittende huid of verslapping van de buik (ook al is de stoma goed gepositioneerd) biedt weinig ondersteuning voor de stoma.
  • Ileostoma met high output of urinestoma  Wanneer de stoma-afscheiding meer dan 750 milliliter per dag bedraagt, neemt de kans op lekkage toe. Hetzelfde geldt als de output meer vloeibaar is.

Vloeibare ontlasting en urine sijpelt gemakkelijker in kleinere huidplooien en tast de huidbarrière sneller aan, zelfs bij stoma's op huidniveau. Als hier sprake van is kan een convex huidplaat de draagtijd van het stomamateriaal verlengen. [2,11]

Verschillende convex oplossingen om aan de behoeften van de stomadrager te voldoen

Afhankelijk van de individuele behoeften is er de mogelijkheid om te kiezen tussen standaard of zachte convex stomamateriaal met diepe of gemiddelde convexiteit. B. Braun heeft een hele reeks convex opties om aan de behoeften van de patiënt te voldoen. Vind een goede oplossing voor elke individuele patiënt.

Geleidelijke overgang in diepte van de convex
Dwarsdoorsnede van een convexe stomabasisplaat met gordelogen voor stomagordelfixatie, een convexe ring, de hellingshoek naar beneden en de diepte.

Een licht aflopende convexiteit is een kenmerk van alle producten van 4 tot 6 mm. Het stomamateriaal moet passen bij de beweeglijkheid van de stomadrager en de kenmerken van de buik van de stomadrager. Daarom zijn er verschillende 4 tot 6 mm convex huidplaten. Ze verschillen in diepte, profiel (helling of hellingshoek), spanning of druk, stevigheid, zachtheid en flexibiliteit. Convex stomamateriaal is ontwikkeld om een betere drukverdeling te bieden en voldoende druk rond de stoma uit te oefenen en zo beter aan te sluiten om de peristomale contouren van de patiënt..

Een stomagordel kan worden gebruikt om het effect van convexiteit te versterken.

Om meer gevoel van veiligheid te creëren, raden veel gebruikers het toepassen van een stomagordel aan. Ook kan een stomagordel het effect van convexiteit versterken. Dankzij drie verschillende bevestigingsposities* zorgen ervoor dat de gordel kan worden aangebracht in een zelf te bepalen voorkeurspositie op de buik van de stomadrager.
